After the dismissal of Steve Bruce from the post last week, the likes of Thierry Henry and Rui Ferreira were linked with the job.
The club took to their website on Wednesday evening to announce that they had decided to appoint Smith, who will leave his job as Brentford manager with immediate effect.
The new head coach of the 'Villains' has been a fan of the club since he was a child, and has the coaching credentials to back up his appointment after making Brentford a proven Championship team challenging for the play-offs since joining in 2015.
As was thought, former England and Chelsea legend John Terry will join as his number two, just days after announcing his retirement from playing.
Terry captained the club to the Play-Off final last season, where they lost out to Fulham, and will hope the club can go a step further this season.
