With Tony Pulis' side without star men Ben Gibson and Adama Traore for one reason or another, it was Millwall who flew out of the blocks on home turf.
The hosts took the lead with just 12 minutes gone as Lee Gregory crossed from the the right for Aiden O'Brien to sweep home low beyond Darren Randolph.
Truth be told, Boro looked shaky without Gibson as youngster Dael Fry partnered summer signing Aden Flint at the back and it was the former Bristol City man who was at fault as Millwall doubled their lead eight minutes before the break.
Flint's headed backpass lacked pace, with Gregory stealing in to round Randolph and slot into the empty net.
Millwall's dominance continued after the break with Steve Morison agonisingly close to a third as he stretched to convert a low centre from the left, with Middlesbrough hardly threatening at the other end.
However, with the clock running down, the Teessiders found another gear, with Martin Braithwaite giving them a lifeline with just three minutes to go as he stabbed home at the far post following good work from substitute Marcus Tavernier.
The usual bout of pressure followed and as the game moved into the seventh minute of stoppage time Boro found a shock leveller. Defender George Friend was the first to react as the ball flashed across the six-yard box as he poked an effort beyond Jordan Archer to secure Middlesbrough an undeserved point.
