Burnley manager Sean Dyche is interested in bringing Cork to Burnley, while the Premier League club have completed deals with Charlie Taylor and Jonathan Walters.
The fee for the midfielder could be worth as much as £10m to Swansea. Cork had two loan spells with Burnley during his Chelsea career, scoring three goals in 40 Championship appearances in 2010/11.
The 28-year-old later joined Southampton on a permanent deal before his move to Swansea in January 2015.
Cork has played for England Under-19 and England Under-21 level, and was also chosen for the 18 man squad for the 2012 London Olympics.
